ACHC Partnership

The Accreditation Commission for Health Care (ACHC) and the Pharmacy Compounding Accreditation Board (PCAB), has partnered with PersonalMed in an effort to ensure the quality and safety of medications within the compounding industry. ACHC has an extensive background and experience in pharmacy compliance and accreditation programs for sterile and non-sterile compounding, infusion, and specialty. This partnership is vital for the credentialing of our network pharmacies and offers a range of resources to help pharmacies achieve PCAB accreditation.

All PersonalMed member pharmacies must be PCAB accredited through ACHC. PCAB accreditation assesses the compounding process on a specific set of standards that focus on the quality and consistency of compounded medications. PersonalMed believes PCAB accreditation demonstrates the level of performance and compliance expected of PersonalMed network pharmacies.

ACHC is a non-profit accreditation organization that has stood as a symbol of quality and excellence since 1986. The organization has CMS Deeming Authority for Home Health, Hospice, and DMEPOS and a Quality Management System that is certified to ISO 9001:2008.

Beginning July 2014, PCAB accreditation became a service of ACHC, offering non-sterile (ref. USP) and sterile (ref. USP) compounding services. PCAB accreditation is the industry’s leading compliance solution and assesses the compounding process based on a specific set of standards that concentrate on the quality and consistency of compounded medications.

PCAB Accreditation

The PCAB Accreditation process is designed to help pharmacies quickly and easily reach their accreditation goals.

 

  • Register at cc.achc.org/PMRX and download program-specific standards
  • Select and coordinate Consulting Service option with Accreditation University.*
  • Submit application and deposit through Customer Central
  • Submit Accreditation Agreement and payment through Customer Central
  • Submit PER Checklist
  • ACHC conducts on-site accreditation survey
  • ACHC’s Clinical Team reviews findings and provides survey results and accreditation decision
  • Pharmacy completes Plan of Correction (if applicable)
  • Pharmacy submits annual verification of compliance

Compliance Standards

Pharmacy Compounding Accreditation Board (PCAB) based on (USP) United States Pharmacopeial Convention standards, offers the most comprehensive compliance solution in the industry. PCAB accreditation assesses the compounding process based on a specific set of standards that concentrate on the quality and consistency of medications produced. PCAB accredits pharmacies that dispense medications pursuant to a prescription order for an individually-identified patient for the following pharmacy services:

  • Non-Sterile Pharmacy Compounding (CFNS)
  • Sterile Pharmacy Compounding (CFST)
